Job description

The Cleveland Clinic Home Care Services seeks an RN to join our Home Care Team serving Cleveland and Northeast Ohio. This PRN role provides assessments, complex treatments, and education to prevent readmissions, with a flexible schedule supporting work-life balance.

Responsibilities include direct nursing care, care plan implementation, supervising LPNs and aides, comprehensive documentation, and cross-disciplinary care coordination. Weekend shifts are required every six weeks.
