Professional Case Management (PCM) seeks a Registered Nurse - RN Case Manager to provide in-home visits in Washington for former Nuclear Weapons Workers with chronic/terminal illness. You will conduct weekly assessments, coordinate care, and supervise caregivers to ensure quality home care.
