Job description
Home Health Care, Inc. is seeking an experienced RN to serve as Float RN Case Manager across multiple Minnesota territories.
The role rotates through service areas based on census and staffing, providing coordinated home health care and comprehensive assessments. You’ll manage assigned caseloads, complete OASIS Start of Care, Recertification, and Discharge, develop plans of care, and collaborate with physicians and interdisciplinary teams.
