Job description
SWLA Center for Health Services seeks a Clinical Float Pharmacist to fill orders, monitor therapies, and provide drug information. Travel between Lake Charles, Crowley, and Lafayette sites is required to deliver clinical coverage.
Responsibilities include MTM, medication reconciliation, patient counseling, and collaboration with providers to optimize outcomes. Requires Louisiana licensure and ongoing CE through ACPE requirements.
