Job description
LCMC Health in New Orleans, LA seeks a Care Manager who serves as the hub of patient case management and care planning. You’ll develop individualized care plans, coordinate with the care team, and drive quality outcomes across the continuum of care.
You’ll document workflows in EPIC, educate patients and families, and ensure safe transitions from hospital or ED. A Louisiana RN license and 2 years of nursing or care management experience are required.
