Job description
Home Health Care, Inc. is seeking an RN Case Manager to float across our 21-county Minnesota service area. This full-time role offers flexible territory rotations, approximately three months per assignment, with on-call shifts and a strong focus on OASIS, care planning, and interdisciplinary coordination.
