Job description
Rising Ground, Inc. in The Bronx, NY, is seeking a Case Planner to provide direct case planning services for children in foster care and connect families with services to address challenges and support reunification.
The role requires collaborating with caregivers, schools, medical staff, and community partners; conducting home visits; documenting progress, and attending Family Court hearings. Full-time hours Monday through Friday in NYC.
