Job description
The House of the Good Shepherd in Utica, NY is seeking a Case Planner to implement family assessment service plans and provide essential in-home services. You will work closely with families, agency personnel, and other providers while maintaining flexible hours and participating in a rotating 24-hour on-call schedule.
The role requires a Bachelor’s degree, UCR documentation experience, and strong collaboration across teams; a valid NYS driver’s license is required.
