Key Responsibilities
Conduct in-depth evaluations of residents to assess their physical capabilities and therapy requirements, ensuring personalized treatment plans are developed.

Utilize therapeutic exercises, manual therapy techniques, and various modalities to effectively treat physical impairments.
Monitor patient progress and adapt treatment plans based on measurable outcomes and feedback.
Work collaboratively by communicating with the interdisciplinary team to develop coordinated care strategies that encompass all aspects of the patient's health.
Educate patients and their families on strategies and home exercises to maintain and enhance physical wellness.
Maintain compliance with state and federal regulatory standards and uphold the organization's best practices in therapy services.
Patient Population / Client Focus
The Physical Therapist will primarily work with a diverse client population in home health settings, including elderly individuals and those recovering from surgery or injury. Understanding the specific challenges faced by geriatric patients or those with complex health conditions is crucial to thriving in this role.
Qualifications
A current and valid Physical Therapist license in the state of New York.
A Bachelor's Degree in Physical Therapy or higher from an accredited institution.
Proficient communication and interpersonal skills necessary for building relationships with patients, families, and professional peers.
Strong organizational skills to balance multiple patient cases and documentation needs effectively.
Preferred Experience
A minimum of two years of experience working as a physical therapist, ideally within skilled nursing facilities or with geriatric populations.

Compensation and Benefits
