This position provides the opportunity to work closely with a skilled interdisciplinary team while supporting patients with medical needs in the comfort of their homes. The CNA will provide services throughout the branch's entire coverage area and will collaborate closely with Registered Nurses, LPNs, Physical Therapists, Occupational Therapists, Speech Therapists, and other members of the interdisciplinary team.Service area includes travel throughout the Shelton Branch coverage area below:Seymour, Ansonia, Derby, Shelton, Monroe, Trumbull, Stratford, Bridgeport, Easton, Fairfield, Weston, Westport, Wilton, Norwalk, New Canaan, Darien, Stamford, and Greenwich.Reliable transportation is required.
