Home Health Physical Therapist Assistant Full TimeHalcyon Home is hiring a Home Health Physical Therapist Assistant Full Time to support our growing geriatric home health population in Fort Worth, TX and surrounding areas. We offer competitive pay and wonderful team support.As a Physical Therapist Assistant, you will provide physical therapy to our patients in both community and residential settings in the Fort Worth, TX area and surrounding areas. A minimum of one year of home health experience is preferred, along with a proven ability to manage a fast-paced, independent work schedule. Knowledge of Homecare Homebase software is a plus!Essential Functions:Performs ongoing clinical assessment, including OASIS assessments, at appropriate time points to identify patient needs, determine level of functioning, and appropriateness of patient for home care.Develops and evaluates the plan of care in partnership with the patient, representative (if any), and caregiver(s).Documents, prepares, and completes clinical notes, phone orders, and other clinical record documentation on an ongoing basis and in a timely manner. Revises the POC based on evaluation and ongoing assessment data.Meets mandatory continuing education requirements of the Agency and licensing board.Assists and instructs patients and families in transfer techniques appropriate to patient abilities. Instructs on use of equipment such as walkers, wheelchair, canes, etc., in a safe manner. Utilizes appropriate therapy equipment specific to patients' diagnoses and needs.Provides patient, caregiver, and family counseling; Provides patient and caregiver education.Provides services that are ordered by the physician as indicated in the plan of care; oversees other team members, when applicable, in the performance of patient care that is consistent with patients' needs.Applies concepts of infection control and standard precautions in coordinating or performing patient care activities to prevent contamination and transmission of disease.Accepts clinical assignments that are consistent with education and competence to care for patients.Provides supervision and guidance to physical therapy assistants and home health aides, as appropriate.Plans and provides appropriate treatment for neuromuscular, neurological, and orthopedic disorders based on medical referral and therapy assessment.Communicates with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care.Requirements:Current PTA licenseKnowledge or experience utilizing Homecare Homebase EMR would be a plusExperience in home health or related healthcare setting preferredStrong clinical and assessment skillsExcellent communication and interpersonal abilitiesAbility to work independently and manage time effectively.What Makes Us Different:People-First Culture: WE truly value and support our team.Excellent Core Values: Communicate, Connect, Collaborate, Care, and Say Yes!Team Support: Work alongside a caring and collaborative group of professionals.Benefits:401(k)Dental insuranceDisability insuranceHealth insuranceHealth savings accountLife insuranceMileage reimbursementPaid time offPaid HolidayTuition reimbursementVision insurance
