Job description
Bridge Home Health in Napa/Solano County seeks an experienced RN Case Manager to plan, organize and direct home care services for a caseload in the community.
You will assess patients, develop and monitor care plans, coordinate with physicians and families, and provide direct nursing care as needed. California RN license, CPR and 2+ years of home health experience are required. This is a full-time position with on-call duties and a focus on community health.
