Full-Time Hospice RN Needed

As a Hospice RN, You'll:
Provide one-on-one care in the comfort of home
Support patients and families with dignity and compassion
Spend meaningful time where it matters most
Care without the chaos of hospital halls
Collaborate with a team to provide thoughtful, individualized care

We’re looking for a flexible RN!
30-40 hours per week
Includes rotation of the on-call schedule
A flexible opportunity for an RN who wants to work around family schedules while continuing to make a meaningful impact through nursing.
As a Hospice RN with AdvisaCare Hospice, you'll provide emotional support and compassionate clinical care, helping patients experience their final days with dignity, comfort, and respect.
You’ll also:
Conduct appropriate supervisory visits
Provide clinical observation and feedback to assigned team members
Participate in interdisciplinary group meetings
Help develop, review, and revise patient/family plans of care
Advocate for the needs and wishes of patients and their families

Requirements
Initial and on-going assessment of patient's physical, functional, psychosocial and environmental needs
Implement the individualized plan of care and revisions as needed
Compliance with AdvisaCare Hospice Policies and Procedures, Medicare compliance
Consulting with and educating the patient/family on disease process, self-care techniques, end-of-life care, dealing with issues of ethical concern
Supervision of paraprofessionals providing services to patient
Preparing clinical and progress notes, participating in IDG conferences
Participate in Hospice performance improvement program
Maintain high level of customer satisfaction with our clients, families and caregivers
Communication of information using current process and technology within organization
Current Michigan License in good standing
Minimum 1 year hospice experience
Empathetic approach to patients and caregivers
Participate in field visits daily, weekly office meetings as indicated
Ability to adapt to patient's status and needs on a full time on call basis
