Job description
Three Oaks Hospice, a network including Agape Hospice Care and Sage Hospice, seeks a full-time RN to provide skilled nursing care in patients’ homes across Clarke County and surrounding areas. You will coordinate care with the IDT, perform regular assessments, develop and update the plan of care, and educate patients and families about end-of-life care.
On-call duties may be required. Applicants should have an RN license, a nursing degree, CPR, and at least two years of clinical experience;
