nLocation: Veterans and First Responders Treatment Program - Saranac Lake, NY
nPosition: Full-time Peer Support Case Manager
nShift/schedule: Tuesday-Saturday, 2:30pm-11:00pm
nPay Range: $18.50/hour to $21.00/hour
nPosition Summary: Provides peer support services and under the direct supervision of a Qualified Health Professional, functioning as a case manager responsible for the effective case management of an appropriate caseload.
nEducation and Training: High School Diploma and CRPA/CRPA-P Certification.
nQualifications and Requirements: Experience working with diverse populations in a trauma informed environment and knowledge of outside community sources. Detail oriented with professional communication and teamwork skills. Proficient in word processing/computer operations and telephone systems. Adhere to Sanctuary commitments. nPhysical Demands: Some lifting of light weights. Ability to stand in working positions.
nEssential Duties:
nnnDevelop and maintain therapeutic relationship with residents.
n
nnAssisting in developing recovery and aftercare planning with residents in the program.
n
nnAssisting residents in becoming aware of existing social and other support services in the community.
n
nnModeling coping skills.
n
nnProviding residents transportation to medical appointments, engaging in recreational activities to include walking/hiking/sporting events.
n
nnWorking with residents to identify strengths that will aid them in their recovery efforts.
n
nnLink residents to formal recovery supports.
n
nnTravel training – informing residents about local transportation options and how to best access and utilize them.
n
nnActively participate in clinical supervision.
n
nnFunction as a supportive multi-disciplinary team member.
n
nnSupport resident retention initiatives.
n
nnFacilitate didactic lectures related to chemical dependency and associated subjects.
n
nnFacilitate and document group therapy sessions.
n
nnComplete thorough admission procedure for residents.
n
nnComplete accurate and thorough discharge summaries.
n
nnProvide biweekly case management sessions for residents.
n
nnDocument monthly treatment plan reviews per compliance regulations, clearly identifying treatment received and progress demonstrated.
n
nnComplete accurate and timely documentation of case management responsibilities.
n
nnDemonstrate consistent familiarity with and sensitivity to military culture.
n
nnDisplay professional image, appearance, and behavior rejective of SJATRC Mission, policies, and procedures.
n
nnAssure compliance with all necessary regulations.
n
nnActively participate in meetings, committees, and quality assurance actives.
n
nnMaintain continued professional education.
n
nnMaintain NYS Licenses and/or credentials as appropriate.
n
nnPerform other duties as assigned by the Program Director.
n
nnConduct observational rounds of facility thought-out shift.
n
nnProvide conflict resolution support for and between residents.
n
nnProvide support and counseling to residents other than assigned caseload as needed.
n
nnRespond to emergency situations in accordance with SJATRC policies and procedures.
n
nnCommunicate pertinent clinical information to other staff through documentation.
n
nnAssess and monitor residents experience signs and symptoms of withdrawal using appropriate withdrawal scales.
