n n n We are seekingn n n n n threen n n n n Pn n n ediatric Advanced Practice Cliniciansn n n n n n n to joinn n n ourn n n n n Virtual Practice Support and Extended Caren n n n n n n multisite practicen n n .n n n Clinicians are expectedn n n to live in the greater Charlotte, NC, arean n n , one of the fastest growingn n n areas in the countryn n n .n n n n n n n
n n Position Details:n n
n n Virtual Practice Supportn n n (VPS)n n n n n :n n n Then n n n n APPn n n team will be affiliated withn n n multiplen n n “brick and mortar” practices, becoming an extension of those clinicians. Providing inbox coverage for clinicians who are out of office, managing refill requests, performing virtual visits andn n n eVisitsn n n , and taking care of chronic conditionsn n n during the dayn n n , then n n APPn n n will “support” their affiliated practice colleagues to truly disconnect when they are not in office,n n n maintainingn n n seamless care to patients and families. Thisn n n practicen n n is staffed byn n n Advanced Practice Clinicians, supported by the physician team.n n n These APPS may also provide in-office visits as coverage for providersn n n outn n n of the office.n n n n n

n n Extended Care (EC):n n n n n n n IN the evenings and on weekends,n n n APPs will transition to pn n n rovidingn n n sharedn n n IN-PERSON aftern n n -n n n hours caren n n n n 7 days a weekn n n .n n n Then n n n n teamn n n willn n n thereforen n n be able to have a mix of both virtualn n n /remoten n n and direct patient care.n n n The Extended Care location will be open from 5-8pm M-F andn n n 9n n n a-n n n 5n n n p on Sat and Sun.n n n n n
