Physical Therapist

Job Title: CWHH-Physical Therapist
Location: Lady Lake, FL
Setting: Home Health

Contract Length: 13 weeks.
Candidate Type: Traveler
Requirements: Current State License as a Physical Therapist; Proof of current CPR certification; Current driver's license. Must have reliable transportation and insurance; 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting.
Responsibilities:
Perform clinical assessment to identify patient needs and determine appropriateness of care.
Document, review, and complete a Plan of Care (POC), progress notes, phone orders, and other clinical records; revise the POC based on evaluation and ongoing assessment data.
Collaborate with interdisciplinary team and work in consultation with physicians.
Plan and provide treatment for neuromuscular, neurological, and orthopedic disorders based on medical referral and assessments.
Assist and instruct patients and families in transfer techniques and use of equipment such as walkers, wheelchairs, canes, braces, prosthetic devices, and appliances.
Educate patients and families regarding therapeutic procedures, considering the patient's age, condition, culture, and learning readiness.
Apply infection control concepts and universal precautions during patient care activities.
Accept clinical assignments appropriate to education and competence to ensure quality patient care.
Perform other related duties as assigned.
