Job description
PruittHealth Corporation is seeking a Nurse Practitioner to deliver compassionate, patient-centered care for D-SNP members. You will perform comprehensive exams, manage acute and chronic conditions, and coordinate care across the care team in home and telehealth settings.
Responsibilities include medication reconciliation, documentation in EMR, and participation in QAPI and quality initiatives to support member health outcomes.
