Job description
ChenMed LLC seeks a Nurse Practitioner to provide direct patient care across ChenMed centers, SNF and home settings. You will conduct geriatric assessments, develop nursing care plans, document progress, and refer to specialists as needed.
You will advocate for patients, coordinate with the multidisciplinary team, and may perform invasive procedures after competency. Extensive travel may be required to meet patient needs.
