Job description
TruHealth is seeking an advanced practice provider (NP or PA) to deliver personalized care in nursing homes and assisted living communities enrolled in Medicare Advantage plans. You will collaborate with a team of providers, case managers, and facility staff to improve quality of life and reduce hospital visits.
The role requires an APRN or PA license (FNP/AGNP/AHACNP or PA), a geriatric focus, and 3 years of clinical nursing or rehab experience.
