Job description
TruHealth seeks an advanced practice provider (NP/PA) with geriatrics focus to enhance outcomes for long-term care residents. You will work with a team of providers and case managers to deliver coordinated, high-quality care in nursing homes and assisted living communities.
You will operate with independent judgement, manage acute and chronic conditions, and collaborate with facility partners to implement TruHealth's national model within regulatory guidelines.
