Job description
TruHealth is seeking an advanced practice provider, either a nurse practitioner (NP) or physician assistant (PA) with a geriatrics focus, to improve outcomes for residents in long-term care settings. You will collaborate with a team of providers and case managers to deliver coordinated care.
You will work primarily in nursing homes or assisted living communities serving residents enrolled in Medicare Advantage plans, exercising independent judgment with supervision as appropriate.
