Job description
TruHealth is seeking an advanced practice provider, NP or PA, with primary care or internal medicine experience and a geriatrics focus. You will work primarily in nursing homes or assisted living communities, delivering coordinated, patient-centered care to reduce hospital visits.
You will assess medical and mental health needs, prescribe medications, order labs, and collaborate with RNs and supervisors to implement the national care model.
