Job description
TruHealth in Iowa is seeking an advanced practice provider (NP or PA) with geriatric focus to improve outcomes for long-term care residents. You will collaborate with a team of providers and case managers to deliver coordinated, patient-centered care within nursing homes or assisted living communities enrolled in special Medicare Advantage plans.
