Job description
Reliant Medical Group, part of Optum, is seeking a Nurse Practitioner or Physician Assistant for a Skilled Nursing Facility in Leominster, MA. The 0.6 FTE role (24 hours/week) focuses on geriatrics across skilled rehab, long-term care, and palliative care, working with physicians and interdisciplinary teams.
The APC will manage complex geriatric conditions, participate in goals-of-care discussions, and contribute to a collaborative, value-based care model with competitive benefits.
