Job description
TruHealth is recruiting an advanced practice provider (NP or PA) with geriatrics focus to join our team in long-term care settings. You will deliver coordinated, patient-centered care to residents in nursing homes and assisted living communities enrolled in special Medicare Advantage plans.
You will collaborate with physicians, care managers, and facility staff to implement our national care model, exercising sound clinical judgment while ensuring regulatory compliance and quality outcomes.
