Job description
TruHealth is seeking an advanced practice provider (NP or PA) with geriatric-focused experience to improve outcomes for residents in long-term care settings. You'll work with a team of providers and case managers dedicated to proactive, preventative care.
You will exercise independent judgement, treating acute and chronic conditions while coordinating with facility partners and health plan leadership to implement TruHealth’s national care model in compliance with regulations.
