Job description
CareMed in Ohio seeks a Nurse Practitioner to provide primary and preventative care for residents in long-term care facilities. You will manage acute changes in condition and participate in advanced care planning and chronic disease management.
The role requires prescribing medications under OH Law, coordinating treatments, and regular communication with nursing home staff and the primary care physician. Experience with EMR and geriatric care is preferred.
