Job description
TruHealth is seeking an advanced practice provider (NP/PA) with primary care or internal medicine experience, preferably in geriatrics, to join our team serving residents in nursing homes and assisted living communities enrolled in Medicare Advantage plans.
You will collaborate with physicians, RN case managers, and facility partners to deliver personalized, coordinated care that improves quality of life and reduces hospital visits, while following federal and state regulations.
