Job description
TruHealth is seeking an advanced practice provider (NP or PA) with geriatrics focus to improve outcomes for long-term care residents. You will work with a team of providers and case managers to deliver coordinated care within nursing homes and assisted living communities enrolled in Medicare Advantage plans.
You will exercise independent judgment, consult with a supervising physician as needed, and contribute to TruHealth’s national care model while complying with state and federal regulations.
