Job description
Senior Community Care of Jefferson County in Louisville, KY is seeking a Nurse Practitioner to provide primary care to PACE enrollees under the supervision of the Executive Director and Medical Director. The role includes performing assessments, care planning, and coordinating with the IDT to support frail elder residents.
Qualifications include RN license, NP certification with prescriptive authority, and geriatric NP preferred.
