Job description
SCAN Health Plan seeks a Nurse Practitioner or Physician Assistant to join our patient-facing team in a remote/field capacity serving the Santa Barbara area. You will collaborate with primary care physicians and specialists to provide high-quality, coordinated care for our members.
The role involves exams, vaccine administration, chronic disease management, and goals-of-care discussions, with documentation and care-gap closure a constant focus. Travel ~75% and HIPAA compliance are essential.
