Job description
Chatham Hills Subacute Care Center in New Jersey is seeking a skilled Occupational Therapist to join our rehabilitation team. You will assess referred patients, plan direct care, and develop treatment programs aligned with physician guidance.
Responsibilities include goal setting, documentation of patient progress, education for patients and families, supervising assistants, and collaborating with the care team to deliver patient-centered therapy.
