Job description
Metropolitan Jewish Health System in New York is seeking a Nurse Practitioner or Physician Assistant to provide and coordinate primary care for residents in long-term care settings. You will work with physicians, case managers and facilities to develop care plans and deliver preventive services in line with member goals.
The role requires NP/PA licensure in NY, strong geriatric experience, and proficiency with EMR and care-management tools.
