Job description
St. Elizabeth Physicians seeks an Experienced Geriatric APP to join PACE and oversee community care for enrollees. You will perform CMS regulatory visits with the attending physician and share clinical information with the interdisciplinary team to ensure optimal care.
Working with eight counties in Northern Kentucky, you will coordinate care with physicians, families and community partners and help develop treatment protocols in collaboration with the medical director.
