About this opportunity

NPHire lists this Geriatric Pain Management - NP/PA opportunity in sacramento, California. Review the employer’s description below for duties, qualifications and application requirements.

Job description

Turn a few hours a week into meaningful extra income while serving patients who badly need care. This growing geriatric pain management practice is expanding into the Bay Area and is looking for Nurse Practitioners and Physician Assistants to round on residents in skilled nursing facilities. Clinicians would travel to facilities in Sacramento and Richmond, CA. This is an independent contractor opportunity, not a full-time or part-time job, built for clinicians who want to earn more on top of a primary role. You'd see the same residents week after week, checking in on their pain, adjusting treatment, and coordinating with facility staff. Providers can earn about $4,000 a month (roughly 400 visits at $10 each), and most finish their rounds in just 4-5 hours.

What Makes This Role Different

Earn on your own terms: fee-for-service pay of $10 per patient visit, which works out to roughly $90-150 for each hour spent seeing residents. Most providers see 10-15 residents per hour, and some facilities have two residents per room, so rounds move quickly. Fits around your full-time job: see patients before clinic, after clinic, on weekends, or on your days off, then go on with the rest of your day. Focused visits: you assess each resident's pain and move on, with no full head-to-toe exam. Malpractice insurance with tail coverage is included. You're added to the practice's malpractice plan and stay protected against claims filed even after you leave. A formal onboarding program, so you're supported from your first visit. New graduates are welcome, especially those with an RN background in ortho, pain management, SNF, or high-volume ER. A true ground-floor opportunity in a growing Bay Area market, with more positions opening as additional facilities come on board. Deeply needed work: pain in older adults in skilled nursing facilities is chronically undertreated, and you'd be the provider who finally manages it properly.

Key Responsibilities

Assess each resident's pain during focused rounding visits, including location, severity, and response to current treatment.

Adjust individualized treatment plans combining pharmacological, non-pharmacological, and interventional approaches for acute and chronic pain.

Prescribe and manage pain medications, including controlled substances, in line with state and federal regulations.

Coordinate referrals for physical therapy, acupuncture, spinal blocks, and other interventional techniques.

Educate patients and families on pain management strategies, coping mechanisms, and lifestyle changes.

Collaborate with physicians, physical therapists, and other providers to keep care coordinated.

Maintain accurate, timely documentation in TEBRA and PointClickCare.

Build strong working relationships with leadership and staff at each partner facility.

Qualifications

Active, unrestricted California NP or PA license.

AGACNP-BC, AGPCNP-BC, FNP-BC, or PA-C certification.

Comfortable prescribing and managing medications, including controlled substances.

Active DEA registration.

Able to travel between facilities across the San Francisco area.

Strong communication skills, compassion, and critical thinking.

Background in pain management, orthopedics, physiatry, skilled nursing facility (SNF) rounding, or hospice with controlled-substance prescribing.

New graduates are welcome if their RN background includes ortho, pain management, SNF, or high-volume emergency care.

County or trauma ER experience is a big plus, since you'll already be used to the pace.

Compensation and Benefits

Independent contractor (1099), fee-for-service: $10 per patient visit.

Most providers complete their rounds in about 4-5 hours (an average of 40 patients per day, roughly $400).

Malpractice insurance, including tail coverage, through the practice's plan.

Formal onboarding program.

Flexible schedule that fits around your primary role.
