Job description
BoldAge PACE seeks a compassionate Physical Therapist to provide comprehensive rehabilitation services to older adults in our innovative PACE program. You will assess, plan, and deliver individualized treatment to improve mobility, function, and quality of life across clinic, home, and community settings. Working within an interdisciplinary team, you’ll collaborate closely with physicians, nurses, social workers, and caregivers to coordinate care.
