Job description
Chatham Hills Subacute Care Center is seeking an Experienced Physical Therapist to join our facility. The PT will assess referred patients, direct rehab services, and develop treatment plans aligned with physician guidance.
Responsibilities include documenting progress, educating patients and families, and supervising Physical Therapy Assistants while collaborating with the rehab team. Prior geriatric/long-term care experience is preferred.
