Job description
The Chatham Hills Subacute Care Center seeks an Experienced Physical Therapist to assess and treat patients requiring rehabilitation in a geriatric long-term care setting. You will deliver direct patient care, determine goals according to physician direction, and document patient progress.
You will supervise Physical Therapy Assistants, collaborate with all disciplines, ensure confidentiality and professional conduct, and participate in PPS scheduling and documentation.
