Job description
The Chatham Hills Subacute Care Center is seeking an Experienced Physical Therapist (PT) for our facility. The PT will assess referred patients requiring rehabilitation, develop treatment plans, and provide direct patient care in a geriatric long-term care setting.
The role requires a licensed PT in the state, graduation from an accredited PT program, and experience supervising Physical Therapy Assistants, with strong documentation and multidisciplinary collaboration.
