Job description
The Chatham Hills Subacute Care Center is seeking an experienced Physical Therapist to join our rehabilitation team. The PT will assess referred patients, develop and oversee treatment plans, and coordinate with physicians to optimize functional outcomes for residents.
You will supervise Physical Therapy Assistants, ensure accurate documentation, and educate patients and families. A valid PT license and accreditation are required, with preferred experience in long-term or geriatric care.
