Job description
Wells Health in Utica, NY, seeks a Primary Care NP to provide comprehensive medical management for residents in Assisted Living Programs across the Utica/Rome area, bringing the doctor’s office to the patient and focusing on treating in place to minimize ER transfers.
You will cover 2–4 sites daily, perform exams, manage chronic conditions, coordinate care with staff and families, and ensure compliant documentation in NYS DOH and Medicaid systems.
