Job description
Senior Community Care seeks a compassionate Nurse Practitioner to deliver primary and preventive care to seniors enrolled in our PACE program. You will perform assessments, diagnose and manage acute and chronic conditions, and develop individualized care plans.
You will collaborate with physicians, nurses, social workers, therapists, and caregivers to support safe, independent living within a holistic, person-centered framework.
