Job description
BoldAge PACE seeks a compassionate PCP Nurse Practitioner to provide primary care to older adults in a comprehensive, team-based PACE setting. In this role, you will conduct assessments, manage chronic and acute conditions, coordinate interdisciplinary care plans, and support participants and families in care decisions. You’ll collaborate closely with physicians, nurses, social workers, and therapists to deliver high-quality, person-centered care.
