Job description
Welcome Health is seeking a clinic-based primary care physician to serve older adults in Los Angeles. The role includes home visits, in-clinic care, and telehealth, with a focus on building lasting patient relationships and delivering person-centered care.
You will lead a multidisciplinary team, coordinate with caregivers, document in EMR, provide after-hours coverage, and participate in ongoing quality improvement initiatives to achieve high standards of care.
