Job description
Welcome Health is seeking a qualified primary care physician to provide clinic-based and home-based geriatric care in the Los Angeles area. You will deliver comprehensive annual assessments, manage chronic conditions, and guide care transitions while coordinating with a multidisciplinary team and leveraging telehealth as appropriate.
