Job description
Provide advanced primary and specialty care to geriatric and medically complex patients in their homes and care settings. Conduct evaluations, diagnose and manage conditions, develop individualized treatment plans, coordinate interdisciplinary care, and provide clinical oversight and mentorship to advanced practice clinicians.
