Job description
WelbeHealth in California is seeking a Primary Care Provider to deliver comprehensive medical care to participants enrolled in the PACE program. You will work with WelbeHealth physicians and the IDT to evaluate, plan, and educate participants and families.
This role emphasizes a smaller patient caseload to foster long-term relationships, and collaboration with therapists and other disciplines to coordinate care across settings, with a compensation package including base pay and bonuses.
