Job description
Hartford Healthcare seeks a Geriatric Primary Care provider to deliver compassionate, comprehensive care to older adults across our Connecticut network. In this role, you will manage chronic conditions, perform routine exams, coordinate referrals, and create individualized care plans that promote safety, independence, and quality of life. You’ll collaborate closely with an interdisciplinary team, leverage advanced technology and EHRs, and participate in quality and population health initiatives while growing professionally in a supportive, inclusive culture.
