Job description
Provides direct patient care to adults aged 50+ in a supportive behavioral health setting, emphasizing meaningful relationships and collaborative team work. Responsibilities include assessing, diagnosing, planning, implementing, and evaluating individualized care plans in accordance with nursing standards. Promotes patient and family-centered care by educating on illness management, medication, and post-treatment needs. Ensures safety, quality, and regulatory compliance while addressing patients’ physical, psychosocial, emotional, and cultural needs. Uses critical nursing skills to evaluate patient conditions and collaborate with the interprofessional team to communicate care plans effectively.
Qualifications & Skills:
Required: Nursing Diploma/Associate’s Degree, RN license
Preferred: Bachelor’s Degree in Nursing, 2+ years of experience
Strong communication, assessment, and critical thinking skills
Ability to work safely and maintain alertness in a safety-sensitive role
Key Duties:
Provide compassionate, patient-centered care
Develop and document personalized care plans
Educate patients and families on health management
Participate in quality improvement and regulatory activities
Assess and respond to patients’ physical and emotional needs
